Reasonable Accommodations

If you are an applicant or participant with one or more disabled household members, you may request a reasonable accommodation to any of OHA's rules, policies, practices or services in order to take full advantage of the programs and services OHA offers.

How do I request a reasonable accommodation from OHA?
If you have a disability you may request a reasonable accommodation by filling out OHA's Reasonable Accommodation Request form and returning it to us. OHA will send this form to your doctor or other service provider to verify your need for the accommodation.

If you are requesting an accommodation during your eligibility process for housing assistance please return your Reasonable Accommodation Request form to:
